Privacy Policy
How the MFI Church Health Initiative handles what you tell it — in plain language, and matched to what the site actually does.

This policy covers mfichurchhealthinitiative.org, the church health assessment operated by Ministers Fellowship International (MFI), of 9200 NE Fremont St, Portland, OR 97220, United States. MFI is the data controller for the information described here. This policy explains what we collect, why, who can see it, how long we keep it, and how to have it removed.
The assessment measures the health of a church — its leadership, teams, model, and congregation. It does not ask about anyone’s medical or physical health, and we do not collect health data.
What we collect
When you take the assessment, we ask for:
- Your name
- Your email address
- Your role in the church — pastor, leader, staff, volunteer, or member
- Your church’s name, city, and region
- Your answers to the assessment statements
We do not ask for a password, a payment method, a date of birth, or a mailing address. We set no advertising trackers and embed no third-party content such as videos or maps.
Why we collect it
- Your answers produce your results and, once enough people from a church have taken part, that church’s blended picture.
- Your name and email are used to send you your results and to let you find them again later. That is all. We do not add you to a newsletter or any marketing list from this form.
- Your church, city, and region group your answers with the rest of your team and let MFI understand the fellowship region by region.
Who can see your answers
Your answers are reported anonymously. The MFI administrator console shows blended scores for a church and individual responses stripped of identity — your name and email are never displayed alongside your answers, not to your team and not to MFI. The same is true of every data export the console produces.
A church’s blended picture only unlocks once enough people from that church have responded, so a single person’s answers cannot be read off a small group.
To be fully straight with you: that promise describes MFI’s reports and exports. The underlying database does hold your name next to your submission, and a small number of technical administrators who maintain the service have direct access to it. They use it to run and support the assessment, not to review or report on individual answers.
Who we share it with
MFI may share assessment findings outside the organisation — with partner ministries, in teaching, or in reporting on the health of the fellowship. When it does, only aggregated or de-identified results are shared. Names and email addresses are never included.
We sell your information to no one, and we do not share it for advertising. The only other companies that touch your data are the service providers that run the site:
Supabase
What it does: Database, and sign-in for the admin console
What it sees: Assessment answers, your name and email, church details
Vercel
What it does: Website hosting and performance measurement
What it sees: Request logs, including IP address, and aggregate page-speed data
Resend
What it does: Sending your results email and your results-recovery email
What it sees: Your name, your email, and a link to your results
Google Analytics, through Google Tag Manager
What it does: Anonymous usage statistics — only if you accept analytics cookies
What it sees: Pages viewed, approximate location, device and browser type
MFI is a global fellowship, so these providers may store and process data in the United States and in other countries. By taking the assessment you understand that your information may be transferred outside your own country.
Cookies
Analytics and marketing cookies are switched off until you choose “Accept all” in the banner at the bottom of the screen. Choosing “Necessary only” leaves them off, and the site works exactly the same either way.
Consent record

Why: Remembers the cookie choice you made so we do not ask again. Stored in your browser’s local storage, not sent to us.
Session cookies
Type: Necessary
Why: Keeps an MFI administrator signed in to the admin console. Only set if you sign in as an administrator.
Google Analytics
Type: Analytics — off by default
Why: Counts visits and shows which pages get used. Only set after you choose “Accept all”.
To change your choice, clear this site’s data in your browser settings and the banner will ask again.
How long we keep it
We keep responses indefinitely, so that a church can retake the assessment later and compare where it has grown. There is no automatic deletion schedule. If you want your response removed, ask us and we will delete it — see below.
Your choices and your rights
You can ask us to:
- send you a copy of what we hold about you;
- correct it, if something is wrong;
- delete your response and your contact details;
- stop using your information, or withdraw a consent you previously gave.
Depending on where you live — including the UK, the European Economic Area, and several US states — you may have these rights in law. We honour the requests above regardless of where you live. Contact us at info@mfiglobal.org and we will respond. We will not treat you differently for asking.
Children
The assessment is for adults serving in a church. Everyone who submits a response must tick a box confirming they are 18 or older, and we keep a record of that confirmation. We do not otherwise verify age, so we cannot guarantee who fills it in. We do not knowingly collect information from anyone under 18. If you are a parent or guardian and believe your child submitted a response, contact us at info@mfiglobal.org and we will delete it.
How we protect it
Data is held in a managed, access-controlled database, transmitted over encrypted connections, and reachable by only a small number of named administrators. Administrators sign in with a one-time link sent to an approved email address — there are no shared passwords. No system is perfect, and we cannot promise absolute security.
This is a beta
The assessment is still being built in the open, and parts of it will change. When a change affects what we collect or how we use it, we will update this page and move the date at the top.
